Request Your Win/Loss Statement

You may manually request a Win/Loss Statement form to be mailed to you by downloading and submitting our Win/Loss Request form below.

PLEASE NOTE: Only Win/Loss Statements for the most current year past can be immediately printed from your myBoardingPass online account. (Create or Log Into your myBoardingPass account.) Furthermore, any Win/Loss Statement requests for years prior to the most current year past must be made manually by completing the below form.

Win/Loss Statements will be sent out in the order they are received once the current tax year ends. Completed forms can be brought to the Rewards Center at any Station Casino, Fiesta Casino or its subsidiaries. Please have valid photo identification with you when dropping off the form. The forms sent via mail must be completed in their entirety and signed in the presence of a Notary Public. Station Casinos LLC must receive the original form sent to us via mail or in person before we can proceed. No photocopies or facsimiles will be accepted.
